No medical form necessary for new hires within the first 60 days of employment or orientation (for an amount up to 4x your annual salary with a maximum of $300,000.) Once 60 days have passed, a short medical questionnaire is required in the policy application. This medical questionnaire will also be required in order to increase Employee and/or Spouse/Domestic Partner coverage amount.
